Selenium deficiency during total parenteral nutrition--a case report.

چکیده
THE trace element selenium has recently been recognised as an essential constituent of the enzyme glutathione peroxidase (GSHPx, EC 1.11.1.9) which is thought to have a major function in the removal of hydrogen peroxide and other organic hydroperoxides generated during oxidative metabolism.' Both selenium and vitamin E appear to protect against a myopathic condition seen in young animals put to pasture in spring after being fed indoors during the winter; this condition has been observed in England, Scotland and Northern Ireland.2 Keshan disease is a fatal cardiomyopathy seen in children who live in a region in China where selenium levels are low in staple foods.3 There have been few reports of selenium deficiency during long-term total parenteral nutrition.4 5Currently available intravenous nutritional solutions contain negligible amounts of selenium.5

Concentration of selenium in plasma and erythrocytes during total parenteral nutrition in Crohn's disease.
Plasma- and erythrocyte-selenium concentrations were determined in five consecutive patients with Crohn's disease given preoperative total parenteral nutrition - nil per os - for a mean period of 34 days per patient. No blood components were administered during the total parenteral nutrition. Before the total parenteral nutrition the plasma-selenium level and, to a less extent, the erythrocyte-...

The case of a patient with Crohn's disease complicated by progressive and irreversible encephalopathy, who had been on long-term total parenteral nutrition due to short bowel syndrome, is described. He initially experienced a disturbance of his vision, which was followed by various neurological symptoms during the next 3 years. متن کاملSelenium deficiency, reversible cardiomyopathy and short-term intravenous feeding.
We report the case of a patient with Crohn's disease receiving short-term postoperative parenteral nutrition supplemented with trace elements who nevertheless became selenium deficient with evidence of a cardiomyopathy. This was fully reversible with oral selenium supplementation. Current parenteral feeding regimes may not contain enough selenium for malnourished patients.
متن کاملTitle Acquired acrodermatitis enteropathica after gastric bypass surgery responsive to IV supplementation
Nutritional deficiency is rare in developed countries, but can be acquired from decreased nutrient intake, reduced absorption, and increased gastrointestinal excretion. We report a patient with acquired acrodermatitis enteropathica (AE) who exhibited low plasma zinc levels and concurrent nutritional deficiencies (pyridoxine, selenium and fatty acids). Our patient had undergone Roux-en-Y gastric...
